30 Actual Sentences Found In Patients Hospital Charts

I recently saw this post on a friend’s Facebook timeline, and was compelled to write a response to it. The thing is, most of these chart notes ARE funny, but some are taken out of context. In addition, the language used in several of the notes are completely appropriate when spoken within the medical world, so I clarified those notes in a comment which I posted. Here is the original list of chart comments, followed by my clarifying remarks.

1. She has no rigors or shaking chills, but her husband states she was very hot in bed last night.
2. Examination of genitalia reveals that he is circus sized.
3. Since she can’t get pregnant with her husband, I thought you might like to work her up.
4. The patient is tearful and crying constantly. She also appears to be depressed.
5. The patient has been depressed since she began seeing me in 1993.
6. Discharge status: Alive but without my permission.
7. Healthy appearing decrepit 69 year-old male, mentally alert but forgetful.
8. The patient refused autopsy.
9. The patient has no previous history of suicides.
10. Patient has left white blood cells at another hospital.
11. Patient’s medical history has been remarkably insignificant with only a 40 pound weight gain in the past three days.
12. Patient had waffles for breakfast and anorexia for lunch.
13. Between you and me, we ought to be able to get this lady pregnant.
14. On the second day the knee was better, and on the third day it disappeared.
15. She is numb from her toes down.
16. While in ER, she was examined, X-rated and sent home.
17. The skin was moist and dry.
18. Occasional, constant, infrequent headaches.
19. Patient was alert and unresponsive.
20. Rectal examination revealed a normal size thyroid.
21. She stated that she had been constipated for most of her life, until she got a divorce.
22. I saw your patient today, who is still under our car for physical therapy.
23. Both breasts are equal and reactive to light and accommodation.
24. Patient has chest pain if she lies on her left side for over a year.
25. The lab test indicated abnormal lover function.
26. The patient was to have a bowel resection. However, he took a job as a stock broker instead.
27. Skin: somewhat pale but present.
28. The pelvic exam will be done later on the floor.
29. Patient was seen in consultation by DR. Blank, who felt we should sit on the abdomen and I agree.
30. Patient has two teenage children, but no other abnormalities.

As a physician, I know that some of these chart notes actually make perfect sense to those who work in the medical field and who are on the hospital wards.

For example, “Since she can’t get pregnant with her husband, I thought you might like to work her up.” refers to a physician referring a patient to a fertility specialist who would be able to “work up” a patients to see what the issue might be with respect to difficulty getting pregnant.

Here’s another one: “The patient is tearful and crying constantly. She also appears to be depressed.” Crying “constantly” does not automatically infer that someone is depressed. Ostensibly the patient is distressed over something, but the clinical diagnosis of depression has a set of criteria which must be met on evaluation of the patient.

“The patient has been depressed since she began seeing me in 1993.” Really, just stupid. This patient had depression beginning at the very latest in 1993, and this clinician had begun seeing the patient at that time. So snicker all you want, but the clinician’s presence in the patient’s life is NOT the causative factor in her depression.

“The patient was to have a bowel resection. However, he took a job as a stock broker instead.” If I could count the number of times a patient was advised to have a surgical procedure, only to evade medical advice, I’d be a millionaire by now.

“The pelvic exam will be done later on the floor.” The “floor” refers to a hospital ward. This patient was most likely evaluated in the emergency room, so the plan was to do a full work-up, including a pelvic exam, once the patient was transferred to a regular bed in the appropriate ward or section of the hospital.

“Patient was seen in consultation by DR. Blank, who felt we should sit on the abdomen and I agree.” The phrase, “sit on the abdomen” means that the clinicians who were evaluating the patient had decided to hold off on any interventions with respect to the abdomen, most likely because they were confident that there was no imminent danger, and no need for surgical intervention.

The remaining chart notes ARE funny, and I could see why lay people find them amusing.
Hope this clears up some of the confusion regarding some of the notes which were perfectly sound within the medical world.

Stress And Your Skin

Did you know that stress can do a number on your skin? You are probably familiar with the stress-induced breakout, but prolonged, chronic stress can also prematurely age the skin. Stressful situations trigger the release of cortisol, adrenaline, pro-inflammatory cytokines, and angiotensin, all of which rob your skin of collagen and weaken its cellular repairing capabilities. Circulation is diverted away from the skin during stressful situations, which explains the dull, pale appearance of the skin in people who deal with constant stress.

Cortisol in particular inhibits the growth of fibroblasts which are responsible for making collagen, so they can’t keep up with the constant breakdown of old tissue. The result is skin which is collagen deficient, resulting in thinner, more wrinkled skin. Free radicals are formed in response to stress, which then damages skin cells and adversely affects the condition of the skin.

So how can you fight the effects of stress on the skin? There are three keys:

1. Exercise:

Every cell in the body benefits from exercise, including skin cells The pores of the skin dilate during exercise, and when you work up a sweat, trapped dirt particles are released. In addition, the increased circulation which results from exercise delivers more oxygen and other nutrients to skin cells, as well as carries away waste products and free radicals.

2. Relax:

We all know how frustrating it can be to have a big event looming, only to have an acne breakout right before the big day. The fact is that the stress associated with preparing for the event can trigger excess oil production in the skin, which clogs pores and sets up the perfect environment for an acne flareup. Even other skin issues, like psoriasis, are linked to stress. However, by practicing relaxation techniques, you can increase blood flow to the skin and offset any triggering factors (such as buildup of oil or free radicals) which contribute to skin maladies.

3. Use topicals:

Some of the best friends for your skin are topical agents such as antioxidants and retinoids. I am a big fan of vitamin C, not only because of its general antioxidant benefits, but also because it has a tendency to lighten up brown spots and impart a brighter appearance to the complexion. Retinoids are great for increasing cell turnover, and are potent anti-aging compounds.

I am always being challenged to pick one career that defines me, and it drives me nuts. When people find out that I am a medical doctor, they struggle with the stereotype of what they expect doctors to be like, in other words, very conservative in dress and demeanor, and without any flavor or personality. Well, I’ve got news for you. I will NEVER be a typical doctor. And please don’t doubt my credentials or schooling. I am NOT a nurse (not that there is anything wrong with this highly respected profession), I am a fully licensed and board certified physician.

A huge project came my way recently, and I was selected for it, only to have the decision-maker flip out over my fitness and modeling images and reverse the decision. I was stunned and dejected, but after reflecting on the whole incident, I began to get angry. Part of the problem was that the decision-maker was a complete hypocrite, pretending to be squeaky clean, but who openly praised one of the dirtiest human beings to ever alight on the entertainment scene. To coin an analogy, at the root of this was a case of the bride being upstaged by another lady wearing white. Mind you, I never intentionally wore white, but hey, my doctor’s coat is white.

I am every bit as much about fitness as I am about medicine, and I shouldn’t have to choose one over the other. I am damned proud of what I have accomplished in fitness, especially because I took things to the next level in my forties, not when I was a young whipper-snapper. If people are confused by the sampler plate philosophy by which I live, too bad. Yes, I am a board certified physician AND a degreed fitness professional, IFBB Pro, certified nutrition coach, writer, sponsored athlete and contest prep coach. I know it’s unusual, but why is that so hard for people to grasp? I mean, here I am, doing all of that, sending a message to the world that no one should have to be one-dimensional and boring.

I don’t hide from myself. I am honest. I have sass, and I speak my mind. I am proud of what I have achieved in my life, and I will NOT hide parts of myself which some overly judgmental people may have a problem with. I am NOT going to apologize for having a sense of humor, for using cuss words here and there (though I don’t use them while seeing patients). I am not going to paint a false picture of who I am. If you don’t like what I am doing, no worries. Move on.

If you find that you are compromising your own vision, dreams, or goals, perhaps you need to re-examine why you are allowing that to occur. If you subscribe to the no limits philosophy, then you would never even consider pulling the reins back. I will always encourage driven people to go for whatever they want, and if it doesn’t fit in with the conventions of one of their chosen careers or hobbies, even better. Break stereotypes and show people what you are made of! Don’t hide all the facets which make you who you are!

Less Is More

Are there benefits to training in minimalist footwear? Studies have demonstrated that training while wearing minimalist shoes results in enhanced performance due to greater foot pliability and a closer approximation to a barefoot stance. Since our feet adjust to wearing raised heels over time, it can be incredibly challenging to keep our heels on the ground while performing exercises like deep squats. Proponents of minimalist footwear may even argue that barefoot is best for heavy weightlifting, but the risks of exposure to microbes or hazardous fragments of glass, metal, or rocks, make minimalist shoes a much more feasible choice while at the gym.

Dysfunctional movement patterns become established throughout the body over time. Conventional athletic shoes can often contribute to ankle and foot dysfunction because they inhibit natural foot action. If you don’t believe it, then try to perform some of your physical activities barefoot and see how well, or poorly, you perform. Chances are that the impaired mechanics in the feet and ankles will make it difficult at best to perform those activities. By gradually switching over to minimalist footwear, an athlete can essentially correct improper foot mechanics and optimize kinetic feedback throughout the body during movement.

Benefits Of Minimalist Shoes

Conventional athletic footwear features a cushioned heel which also can compromise power. In contrast, minimalist footwear features little to no heel, and the soles are very thin and malleable, so they allow the joints in the foot to move and adjust to weight loading movements. In addition, the heels and midfoot make solid contact with the floor, causing more activation in the glutes and hamstrings. Studies have demonstrated that people who wear minimalist footwear develop greater strength in their legs and feet, since more power is transferred from the working muscles, through the feet, and into the movement. Plyometric movements and sprint power will also be enhanced while wearing minimalist shoes.

A 2011 study by Squadrone and Gallozzi assessed the ability of experienced runners to estimate the degree of inversion, eversion, dorsiflexion, and plantarflexion, of a slope surface board placed under their right foot while standing. They found that the degree of proprioceptive feedback which subjects received while wearing Vibram Five Fingers® was much more accurate than in subjects who wore a standard running shoe. In conclusion, cushioned shoes conferred a distinct disadvantage when compared to minimalist shoes during assessment of foot position awareness.

How To Transition Into Minimalist Shoes

Anyone who is transitioning from conventional athletic shoes to minimalist footwear needs to do so gradually, since the body has to adjust to radically different proprioceptive input. There have been cases of metatarsal stress which developed after converting too quickly to minimalist shoes. However, once the transition is made, noticeable strength gains should occur. Try spending about 15 to 30 minutes, twice a day, wearing minimalist shoes around the house. After about a week or two, you can try them out at the gym one to two days a week until you become accustomed to the feel of the shoes. Be aware that your gait and foot stance will probably change as you acclimate to this type of shoe.

Are Post-Workout Carbohydrates Necessary?

If you have been lifting weights for a while, you have probably heard about the importance of ingesting simple carbohydrates during the critical post-workout window. The idea is that the insulin spike which occurs with carb consumption augments protein uptake and thus optimizes muscle building and repair.

However, there is a growing body of evidence which argues against this theory. One study which was published in Nutrition and Metabolism split older male subjects into two groups, one of which consumed only protein post-workout, while the other group consumed the identical amount of protein, with an added carbohydrate source in a 1 to 2 ratio. The subjects in the second group initially had an insulin spike and a greater uptake of protein into muscle tissue, but after several hours, both groups had the same uptake of protein.

Athletes who train several times a day may benefit from the faster rate of protein absorption which accompanies carbohydrate consumption during the post-workout window since they need to keep glycogen stores full for the next workout. However, the average person or athlete who only trains once per day will be able to replenish glycogen levels within a day or two, without any negative effects, as long as enough carbohydrates are consumed in the diet throughout each day. This is great news for individuals who cannot consume large amounts of simple carbohydrates, either due to diabetes or excess weight, because the overall absorption and uptake of protein is unaffected by carbohydrate intake.

Substances like whey protein induce an even bigger spike in insulin than carbs do, and the increase insulin levels from protein alone is adequate to inhibit the muscle breakdown which occurs post-lift. You might also consider employing a carb-cycle diet which consists of a low carb plan intermixed with one or two high carb days per week. The carb spike days will effectively replenish glycogen stores and keep your energy high for those intense lifting days, while the remaining lower carbohydrate days will boost growth hormone production.

Jeanne Calment – World’s Oldest Person

Jeanne Louise Calment 21 February 1875 – 4 August 1997) was a French supercentenarian who has the longest confirmed human lifespan on record, living to the age of 122 years, 164 days. She lived in Arles, France, for her entire life, outliving both her daughter and grandson by several decades.

Calment was born in Arles on 21 February 1875.[2] Her father, Nicolas Calment (28 January 1838 – 22 January 1931), was a shipbuilder, and her mother, Marguerite Gilles (20 February 1838 – 18 September 1924), was from a family of millers. She had an older brother, François, (25 April 1865 – 1 December 1962). Some of her close family members also lived an above-average lifespan, although none lived anywhere near as long as Jeanne: her older brother François lived to the age of 97, her father to six days shy of 93, and her mother to 86.

In 1896, at the age of 21, she married her double second cousin, Fernand Nicolas Calment, a wealthy store owner. Their paternal grandfathers were brothers, hence the same surname, and their paternal grandmothers were also sisters.[5] His wealth made it possible for Calment never to have to work; instead she led a leisured lifestyle, pursuing hobbies such as tennis, cycling, swimming, rollerskating, piano, and opera.[2] Fernand died in 1942 at the age of 73 after suffering from a bout of food poisoning.[6]

In 1965, at age 90 and with no heirs, Calment signed a deal to sell her apartment to lawyer André-François Raffray, on a contingency contract. Raffray, then aged 47 years, agreed to pay her a monthly sum of 2,500 francs until she died. Raffray ended up paying Calment the equivalent of more than $180,000, which was more than double the apartment’s value. After Raffray’s death from cancer at the age of 77, in 1995, his widow continued the payments until Calment’s death.

In 1985, Calment moved into a nursing home, having lived on her own until age 110. At the age of 114, she appeared briefly in the 1990 film Vincent and Me as herself, becoming the oldest actress ever to appear in a motion picture.

Calment’s remarkable health presaged her later record. At age 85 (1960), she took up fencing, and continued to ride her bicycle up until her 100th birthday. She was reportedly neither athletic nor fanatical about her health. Calment lived on her own until shortly before her 110th birthday, when it was decided that she needed to be moved to a nursing home after starting a small fire in her house, caused by a cooking accident, which has been attributed to complications with sight. However, Calment was still in good shape, and continued to walk until she fractured her femur during a fall at age 114 years 11 months (January 1990), which required surgery.

Calment smoked cigarettes from the age of 21 (1896) to 117 (1992),though according to an unspecified source, she smoked no more than two cigarettes per day towards the end of her life. She quit smoking, not because of health reasons, but she couldn’t see well enough to light her cigarettes.

Calment ascribed her longevity and relatively youthful appearance for her age to a diet rich in olive oil, which she also rubbed onto her skin, as well as a diet of port wine, and ate nearly one kilogram of chocolate every week. She also credited her calmness, saying, “That’s why they call me Calment.” Calment reportedly remained mentally intact until her very end.

On 4 August 1997, around 10 AM Central European Time Calment died, aged 122.

A Centenarian Who Broke World Records

Tom Lane

Tom Lane broke records as one of the nation’s oldest Masters swimmers. He exercised every single day, shot golf regularly, but his favorite physical activity, and the one which won him gold medals in Masters divisions, was swimming. a former patent lawyer, Tom Lane set Masters records in the backstroke, breaststroke, and freestyle in three age groups. He went on record stating that his life’s philosophy was, ‘If you can’t beat ’em, outlive ’em.’”

Tom Lane proved to younger people that old age was never an excuse to become lazy and sedentary. He went on record saying that for many people, retirement is when an active life can begin. He didn’t even allow blindness to stop him. When glaucoma robbed him of his sight at the age of 92, he began to have problems with turning at the end of laps in the pool, and would bump his head on the edge of the pool. Instead of quitting, he attached a sponge to his forehead to cushion the blow when he reached the edge..

Tom Lane died at the age of 103 from complications of pneumonia in his home in San Diego in August of 1997, but not before leaving an inspiring legacy and breaking world records in Masters swimming.

Who Is THAT In The Mirror?

This post is dedicated to every single patient and client who has admitted to me that at some point after they began their descent into middle age, they looked in the mirror and no longer recognized the reflection staring back. The aging process can be terribly cruel, and when hormonal levels plummet, the physical changes can appear almost overnight. Taut, supple skin is replaced with saggy skin. A full, thick head of hair is reduced to a thin, lifeless mass. Joints begin to creak and resist natural movement. Brilliant white teeth begin to look dingy. Once full cheeks and lips become deflated. Grey and white hairs sprout and take over the scalp. Back fat and belly fat obliterate a once trim waistline. I could go on, but you get the idea.

None of the changes associated with aging are fun, especially the physical changes which end up making us feel frustrated by a rapidly changing physical landscape. While some folks begrudgingly accept the aging process without considering any interventions, a growing segment of the aging population has turned to everything from adrenal support supplements, to hair dye, to dermal injectable fillers, to surgery, in an effort to fight the aging process every step of the way.

The good news is that there are more anti-aging options than ever before. In addition, there is a greater awareness of the importance of proper nutrition and regular exercise in fighting the aging process. But let’s face it. Many people still want a magic pill or an instant fix, and that is where cosmetic interventions such as fillers and surgery come into play.

Is it cheating to have treatments which make you feel more like yourself? I certainly don’t think so, nor do my patients. Unless you are on a mission to pump your face so full of fillers that you look like a Barbie doll, you can turn to fillers to restore volume which has been lost over the years and STILL LOOK LIKE YOU.

This Is What $54 Worth Of Good Cat Food Looks Like…

For the past several weeks, I have been re-training my European Burmese cat Kazu to eat moist cat food so that she will lose weight. She is a very fussy eater, and it has been a guessing game, trying to determine which foods she will eat. My conclusion is that Kazu will only eat food which has had little to no processing, and is considered “human grade”. This little lady is insisting on a whole food diet, which is fine with me, but the kind of food she likes is expensive!

Today I decided to stock up, so I went to C&C Pet Foods (great place for discounted pet food in Burbank) and purchased 32 three ounce cans, 6 three ounce pouches and 1 six ounce can, which came to a total of $54. That’s pretty pricey if you ask me! However, Kazu has refused other brands of foods, so I have to stick with a small handful of brands (Tiki Cat, Weruva, and B.F.F.) which are not available at stores like Petco or PetSmart.

If you have a finicky pet, I would HIGHLY recommend these brands of pet food, because they are high quality and quite palatable to most pets.

So Many Flavors For My Cat To Refuse

Cats are known for being finicky, and this is certainly the case for both of my female cats, Shima and Kazu. Their constant fussiness is balanced out by my boy, Tenshi, who is one of the most food-motivated cats I have ever known. Tenshi will eat just about anything he can get a hold of, including asparagus, avocado, broccoli, bananas and blueberries.

Over the past month I have been trying to get Kazu to eat moist food and to avoid dry kibble, because she is about 30% over her ideal weight. It has been nothing short of exhausting, trying to figure out what she likes to eat. I have gone to the pet store twice, and have selected a bunch of different varieties and brands of food, though I have focused my purchases on the Tiki Cat brand of moist cat food because it is human-grade, the closest to human table scraps, and highly recommended by her vet.

I have purchased about eight different varieties of Tiki Cat, most of which were tuna varieties with other goodies mixed in, and also purchased B.F.F., Innova Evo, Wellness, and Evanger’s. I also purchased tuna and sardines from the grocery store. Here is the status of my experimentation with different types of food:

B.F.F. – Kazu really loved this food, probably because it was a tuna variety. She ate it pretty readily.

Innova Evo – Kazu completely ignored this stuff.

Wellness – Kazu did not like the varieties we tried (tuna chunks and turkey chunks). All she did was give the broth a couple of licks, then completely refused to have anything to do with the food.

Evanger’s – Kazu didn’t even give this food a second’s worth of interest. To be honest, when I opened the can and got a whiff of the food, I was pretty grossed out myself. As a matter of fact, I have noticed that if I find a food off-putting, Kazu will not like it either.

Tiki Cat – Kazu has liked all the tuna varieties, but she was rather indifferent when I offered her the sardine variety. I will say that I am VERY impressed by the consistency and smell of this brand of food. It is appetizing, without any strange odors. With the chicken varieties, you can see actual flakes of shredded chicken. The meats which are used in the Tiki foods are high quality and unprocessed. This has been the winning brand for Kazu, and though I want to try a couple more brands on her, this may be the standard which she will eat on a daily basis. Some varieties of Tiki Cat are priced at close to $2 for a THREE ounce can, so this stuff isn’t cheap!
